Report an Adverse Patient Safety Event
The purpose and authority of the Utah Patient Safety Surveillance and Improvement Program (PSSIP) is depicted in Utah Administrative Code R429-1 opens in a new tab. Facilities are required to report to the Department all patient safety events within 72 hours of the facility’s determination that a patient safety event has occurred. Patient safety events may be categorized as reportable events with outcome assessed by harm scale, reportable events resulting in permanent patient harm, intervention to sustain life, or patient death; and reportable events referenced by other reporting rules.
Utah uses REDCap opens in a new tab, a secure web application for building and managing online surveys and databases, to collect information about adverse patient safety events that occur within facilities. To report a patient safety adverse event, you must first have a REDCAP Account.
All information collected is confidential and is not used to reprimand facilities for occurrences. The database contains records submitted from 2001 on.

Reporting an Adverse Event
Once you have signed up for an account. You are ready to submit your event using the REDCap system.
For Patient Safety Events:
- Go to the REDCap Record Entry opens in a new tab page.
- Click "Add New Record"
- Enter the information of the new event.
- Click ”Save and Exit Form” at bottom or top right of the page to report the event.
