Signing Up for REDCap Is Easy

Register

Register with Utah Master Directory (UMD). For steps click HERE. To reset your password click RESET. For help with UMD click HELP.

Login

Login to REDCap. You may get a notice saying that your access is denied; this is normal. We recommend that you bookmark the page. See detailed instructions for accessing REDCap opens in a new tab.

Email

Ask your supervisor to send an email to [email protected], indicating that you are authorized to submit patient safety reports on behalf of your facility.

Reporting an Adverse Event

Once you have signed up for an account. You are ready to submit your event using the REDCap system.

For Patient Safety Events:

  1. Go to the REDCap Record Entry opens in a new tab page.
  2. Click "Add New Record"
  3. Enter the information of the new event.
  4. Click ”Save and Exit Form” at bottom or top right of the page to report the event.
